Dealing with Zero Sperm Count (Azoospermia): Advanced Treatments

Understanding Azoospermia Types

Azoospermia is categorized into two main forms:

  • Obstructive Azoospermia: Sperm is produced normally inside the testicles but is blocked from exiting due to obstructions in the vas deferens, epididymis, or ejaculatory ducts (often from infections or prior surgeries).
  • Non-Obstructive Azoospermia: The testicles fail to produce sufficient sperm. This can be caused by genetic conditions (e.g., Klinefelter syndrome), hormonal issues, varicocele, or past chemotherapy.

Surgical Sperm Retrieval: PESA, TESA, and Micro-TESE

For obstructive cases, procedures like PESA (Percutaneous Epididymal Sperm Aspiration) or TESA (Testicular Sperm Aspiration) can extract sperm with a fine needle. For non-obstructive cases, Micro-TESE (Microdissection Testicular Sperm Extraction) uses an operating microscope to carefully search for and retrieve tiny pockets of sperm production within the testicular tissue.

Once harvested, the extracted sperm are combined with eggs via Intracytoplasmic Sperm Injection (ICSI) to fertilize them, offering a high chance of a healthy pregnancy.
