A varicocele is an enlargement of the veins within the loose bag of skin that holds your testicles (scrotum), similar to varicose veins in the legs. It is a major cause of male factor infertility, but it does not mean fatherhood is out of reach.

Understanding Varicoceles: The Anatomical Breakdown
To understand how a varicocele affects fertility, it helps to understand the anatomy of the scrotum and the circulatory system of the testicles. The testicles receive oxygenated blood through the testicular artery, and deoxygenated blood is carried away through a network of small veins known as the pampiniform plexus. These veins serve a crucial physiological function: they act as a heat-exchange system, cooling the warm arterial blood before it reaches the testicles.
Sperm production (spermatogenesis) is highly temperature-sensitive. The human body is kept at approximately 37 degrees Celsius (98.6 degrees Fahrenheit), but the testicles must be kept about 2 to 3 degrees cooler to produce healthy, functional sperm.
When the valves within the veins of the pampiniform plexus fail to function correctly, blood does not flow efficiently back toward the heart. Instead, the blood pools in the veins, causing them to stretch and dilate. This pooling is what we call a varicocele. Because blood is pooling, the local scrotal temperature rises, exposing the developing sperm to constant, elevated heat. This heat stress disrupts the delicate molecular processes involved in sperm creation, leading to lower sperm counts, poor motility, and abnormal sperm shapes.
The Clinical Impact of Varicoceles on Sperm Quality
Varicoceles are not uncommon; they are present in about 15% of the general male population. However, when we look at men struggling with primary infertility—meaning they have never fathered a child—that rate jumps to approximately 40%. For men with secondary infertility (those who have fathered a child in the past but are now struggling to conceive), varicoceles are found in up to 80% of cases. This indicates that varicoceles cause progressive damage to testicular function over time.
The damage manifests in three primary ways on a standard semen analysis:
- Count (Oligozoospermia): The high temperature reduces the overall quantity of sperm produced in the seminiferous tubules.
- Motility (Asthenozoospermia): Heat stress damages the mitochondria in the sperm tail, which supply the energy needed for sperm to swim effectively through the female reproductive tract.
- Morphology (Teratozoospermia): Developing sperm are malformed, resulting in a higher percentage of sperm with abnormal head or tail shapes, which impairs their ability to bind to and fertilize an egg.
Beyond these visible parameters, varicoceles cause significant oxidative stress within the testicular tissue. The lack of proper blood flow causes a build-up of reactive oxygen species (ROS), which damages the sperm cell membrane and causes high levels of sperm DNA fragmentation. High DNA fragmentation means that even if the sperm looks normal under a microscope and is able to fertilize an egg, the resulting embryo may have genetic instability, leading to fertilization failure, poor embryo development, or recurrent early miscarriages.
Diagnosing Varicoceles at Kindle Womb IVF

The examination is typically performed while the patient is standing, and sometimes involves the Valsalva maneuver (bearing down to increase intra-abdominal pressure), which makes the dilated veins easier to detect.
Varicoceles are graded based on their severity:
- Grade 1 (Subclinical/Mild): Not visible, and only palpable during the Valsalva maneuver.
- Grade 2 (Moderate): Palpable while standing, without needing the Valsalva maneuver, but not visible.
- Grade 3 (Severe): Easily visible through the skin of the scrotum, often described as feeling like a "bag of worms."
To confirm the diagnosis, evaluate the exact blood flow velocity, and measure testicular volume (as varicoceles can cause the affected testicle to shrink, a condition called testicular atrophy), we perform a high-definition color Doppler ultrasound of the scrotum. This non-invasive ultrasound provides precise anatomical data that helps us map the ideal treatment pathway.
Treatment Options: Varicocelectomy vs. Assisted Reproduction
Fortunately, a varicocele diagnosis is not a dead end. There are two primary pathways to achieving a pregnancy: surgical repair or assisted reproductive technology (ART).
1. Microsurgical Varicocelectomy
This is a minimally invasive outpatient procedure performed under general or local anesthesia using an operating microscope. The surgeon makes a small incision in the groin area and carefully isolates the dilated, abnormal veins, tying them off or clipping them to redirect blood flow to healthy veins. The use of a high-power microscope is critical, as it allows the surgeon to preserve the delicate testicular arteries and lymphatic vessels, minimizing the risk of complications like hydrocele (fluid build-up around the testicle).
Following surgery, about 60% to 80% of men show significant improvement in their semen parameters within 3 to 6 months, and spontaneous pregnancy rates increase substantially. Surgical repair is highly recommended when there is a palpable varicocele, abnormal semen parameters, and no major female fertility factors present.
2. In Vitro Fertilization (IVF) and Intracytoplasmic Sperm Injection (ICSI)
If the female partner has fertility factors (such as blocked tubes, advanced age, or low ovarian reserve), waiting 6 months for sperm quality to improve post-surgery may not be the best strategy. In these situations, couples can proceed directly to IVF with ICSI.
During an ICSI cycle, the embryologist uses micro-manipulators to select a single healthy, viable sperm from the partner's semen sample and injects it directly into the cytoplasm of a harvested egg. Because only a few healthy sperm are needed for the entire process, ICSI completely bypasses the sperm count and motility issues caused by the varicocele.
To address the issue of sperm DNA fragmentation, our laboratory uses advanced sperm selection techniques, such as PICSI (physiological ICSI) or microfluidic sperm sorting channels, which isolate sperm with the highest genetic integrity. In cases where semen quality is extremely poor or DNA fragmentation is exceptionally high, we can perform surgical sperm retrieval (such as TESA or Micro-TESE) to collect sperm directly from the testicles before they are damaged by the transit through the epididymis.
